Name change letter – Example 3

Joint parental responsibility
(everyone with PR consents to the name change)

Everyone with parental responsibility (PR) must sign a letter consenting to your child’s name change.  You can send either one letter signed by everyone with PR (see example letter template 3a below) or you can send separate letters (see example letter template 3b below).  If you have legally adopted your child, please see example 3c template below.

If those with PR will be signing the same letter, but they do not live together, the address of the other person with PR should be printed underneath their signature.

If you are changing more than one child’s name, please provide a separate letter for each child.

Our fax number and postal address can be found at the bottom of this page.

If you have Microsoft Word for Windows versions 2002, 2003 or 2007 on your computer, you can download an interactive Word template that will make it easy to produce your letter of consent.  Look out for this download icon at the bottom of the letter template Look out for this icon to download an interactive Word document for your letter of consent.  If you do not have Word, please copy and paste the template text into your word processor.
